The Document Challenge
As the electronic medical record (EMR) gains widespread acceptance, the need for improved document management has become increasingly important. Naturally, all of the paper documentation that is stored in the paper chart should also be included in the EMR. That’s why IMPAC has developed multi-dimensional approach to meet the document challenge.

Document Management
Fundamental to IMPAC’s EMR is a full-featured document management system that supports the generation and import of documents generated in Microsoft® Word®, making transcriptions an integral part of the electronic chart. Facilities can generate transcriptions onsite – even inserting information directly from the EMR – or import Word documents generated by offsite transcription services. Document management includes online review, editing, and approval as well as document distribution by email, fax, or print.

Structured Noting
Facilities looking for a solution to minimize or even eliminate the need for costly transcription services can take advantage of IMPAC’s Structured Noting capability. Structured Noting makes accurate patient documentation fast and efficient while ensuring that physicians are properly compensated for their time. Based on positives or normals assigned using a predefined, disease-based template, a narrative of the interview is automatically generated – a complete patient note that eliminates the need for transcription. Now an integral part of the EMR, the structured note can be reviewed, edited, and approved as part of the EMR then distributed to all identified correspondents via email, fax, or mail. And IMPAC Structured Noting prevents coding errors by suggesting an evaluation and management (E&M) code based on the document content.

Patient Assessments
While IMPAC Structured Noting provides a SOAP-based review of systems designed specifically for the oncologist, IMPAC’s patient assessment tool provides an easy and efficient way of documenting any variety of patient data. The system provides hundreds of standard assessment items – vital signs, physical findings, review of systems, chemo- or radiation therapy induced side-effects and quality-of-life indicators – that can be customized and organized into a series of electronic forms or ‘views’ designed to emulate your existing paper forms. But electronic patient assessments actually facilitate clinician review by providing color-coded alarms and warnings that flag laboratory and assessment levels that are out-of-normal range, providing an ‘at-a-glance synopsis of the patient’s condition.

Document Scanning & Import
Because IMPAC knows that even the most paperless office is confronted by the challenge of integrating outside documents into the EMR, document scanning and import is also supported. Electronic document scanning allows facilities to scan external documents such as pathology reports and store them in the electronic chart. IMPAC also provides an HL7-compliant document interface to support the import of documents from other healthcare information systems. Best of all users can categorized imported documents and review and process them along with all other documention in the IMPAC oncology EMR.
